For 2026, the standard monthly Medicare Part B premium is projected to be $206.50, an increase of $21.50 from 2025. The Part B deductible is expected to increase to $288. Please note this is an estimate for Part B, Medicare will release the official amounts in early November.  

  • The maximum Part D deductible for prescription drugs will rise to $615. Additionally, there are new initiatives like the $2,100 cap on out-of-pocket drug costs and the option to spread these costs throughout the year.  
  • Medicare Part B  
  • Standard monthly premium: Projected to be $206.50 (an 11.6% 
  • increase from the $185 in 2025). 
  • Deductible: Expected rise to $288 (a 12% increase from $257).  
  •       Medicare Part D (Prescription Drug Coverage)  
  • Maximum deductible: Will increase from $590 to $615. 
  • Out-of-pocket cap: A $2,100 cap on out-of-pocket costs for covered drugs will be in place. 
  • Prescription Payment Plan: Individuals can opt to spread their out-of-pocket drug costs over the entire year, rather than paying a large sum at once.  
  • Other 2026 changes  
  • IRMAA (Income-Related Monthly Adjustment Amount): Higher-income earners may see increased surcharges on their Part B and Part D premiums. 
  • Medicare Advantage: The Chronic Care Management benefits will have tighter rules. 
  • Drug prices: Prices for 10 high-cost drugs will be negotiated by Medicare, with more price cuts expected in the future. 
  • Insulin: A $35 monthly cap on insulin costs will continue 
  • For Bronx Residents on Medicare Advantage plan: Many plans in Bronx County are showing reductions in ancillary benefits such as OTC, Dental, Vision and hearing. A good amount of plans will have higher co-pays on many medical services including a medical deductible and/or added monthly premium. Please review your annual notice of changes, the insurance company mails this document out in early October. For Bronx Residents on a Medigap Plan and Standalone drug plan: Many members are experiencing rate increases at approx. 20-25% for 2026. Standalone Part D plans are expected to increase in copays/coinsurance for select brand name drugs, with a slight increase in Part D premium. There are less standalone drug plan options for 2026. There were 12 standalone drug plan options in 2025, there are now 10 standalone drug plan options. Please review your Part D annual notice of changes which were sent out in early October.
