Take control of your future health decisions. This essential workshop empowers you with the health literacy needed to understand and complete your Advance Directives. We'll demystify which documents (Living Wills, Health Care Power of Attorney, etc.) need to be filled out, emphasize the critical need to formally appoint a healthcare surrogate/agent WHO WILL SPEAK FOR YOU IF — AND ONLY IF—YOU CAN NO LONGER SPEAK FOR YOURSELF. We will also provide guidance on initiating crucial, respectful conversations with family or friends so your wishes are KNOWN AND WILL BE HONORED. Walk away prepared, with the documents and confidence to protect your future medical choices.

This presentation will be followed with an in-person "Food, Sip and Sign" on Wednesday, November 12, from 10:00 - 11:30, where you will have the opportunity to come to the center to complete the requisite documents, witness them for each other, with RSS social workers and representatives from the MJHS Health System available for clarifications and support.
